Fellowship Senior Living Fellowship Senior Living Basking Ridge, NJ Basking Ridge, NJ Part-time Full-time Per diem Part-time Full-time Per diem $40 - $43 an hour $40 - $43 an hour Under the supervision of the Director of Nursing for Helping Hands, the Homecare Visiting RN is responsible for adhering to the Best Practices established by the Board of Nursing and CAHC Accreditation Standards.
Which includes, supervision of CHHAs, and making monthly follow-up calls/visits to ensure adherence to the Care Plan(s).
S/he will provide input to Director of Nursing with regard to placing aides appropriately with each client.
RN Requirements New Jersey RN license.
Minimum of one year experience in a health care setting, preferably home health agency, hospital, or long-term care facility.
Must demonstrate a broad base knowledge of community health nursing and competent clinical skills.
Demonstrated ability to supervise CHHAs.
Must demonstrate the ability to work within a team setting and possess strong interpersonal skills.
PC literate and demonstrated basic knowledge of computerized charting.
Must be a licensed driver with an automobile in good working condition and have automobile insurance in accordance with New Jersey laws.
Understanding and support of the mission and philosophy of Fellowship Village, Inc.
Supports the goals of the Board of Directors and the administration.
This position will be part of on-call rotation.
Essential Functions Conducts Case Assessments, assists with Care Plan development, and ensures proper completion of initial/follow-up documentation.
Supervises CHHA team members for on-campus/off-campus clients.
Monitors the completion of documentation by CHHA team members.
Maintains Assessment/Care Plan records of current Helping Hands' clients.
Maintains records for State review as required.
Provides direction and assistance to clients/family members regarding Assessments, Care Plans and placement of aides.
Provides in-services to CHHAs who are new to a case or are filling in as a replacement on a case.
Evaluates effectiveness of care provided by CHHAs on (60 day visits) and provides feedback to Director of Nursing.
Participates in care conferences or other meetings as appropriate.
Supervision Supervision of CHHAs in regard to ADL care and providing feedback to the Director of Nursing.
Helps identify team development needs and provides feedback to Director of Nursing.
